United States, the 21st of October 2010: The ex-chairman of the Joint Chiefs of Staff, General Hugh Shelton, revealed the monumental error in his exclusive new book.
The codes are usually held by an aide to the president who remains close to the leader while on his travels.
Gen Shelton advised that there was an incident during the year 2000 when the codes had been lost by the aide and then covered up until such time as they were due to be replaced.
Under the procedures, an official is sent every month to check the codes, which are then replaced every four months as a matter of security.
According to Gen Shelton’s book ‘Without Hesitation’ an official was sent to check the codes in accordance with procedure, before being informed that the president had the codes on his person and was in an important meeting.

The following month a different official was sent to check the codes and was given a similar story. When it came time to change the codes, the aide admitted that the previous codes had been missing for months.
Gen Shelton said that it was apparent that the president had not had the codes and was unaware that the aide had lost them. The general described the episode as a “comedy of errors”.
